Support Ogden Dining

This fund is being organized by the Ogden Downtown Alliance. Please have an owner or general manager complete this form. This fund is open to restaurants, bars and other food service facilities that have closed or have significant change in services due to covid-19.

You will need to provide some contact information for the business and then you will be able to nominate up to five employees for the fund. We are aware that this puts you in an challenging position but we believe that you are the closest to your staff and understand their needs and who is most impacted during this time. Nominations will be kept confidential.

Submitting this application does not guarantee funding. We will be randomly selecting from these submissions to fund employees as money is available.

We will reach out for a W9 and other payment information as needed.

Each employee will be eligible for at most $500, which puts them below the requirements to continue receiving unemployment and other benefits. More information on unemployment eligibility is available here.
